Sports Physicals To Be Given
Return Forms For A Free Physical
Sports physicals for the 2016-2017 school year will be given on Friday, May 6th, 2016. The physicals will be provided free of charge to St. Francis students who will be in grades 7 through 12 for the 2016-2017 school year. All students participating in sports or spirit squads will be required to have a physical before participating in any activity. This is a great opportunity to receive a physical free of charge and complete the requirements for next year.
A signed concussion and physical form must BOTH be on file at the high school before a student is allowed to participate (including practice) in an activity. Please print the forms below, complete them, and return them to Mr. Yates at the high school before May 6th so that the physicals can be conducted. The physicals are FREE! If a student does not return the COMPLETED and SIGNED forms, then the heath physical will not be given. Parents will then need to make an appointment at the clinic and will be charged an office visit.
Tyler Raile will be the provider conducting the physical assessments, Julie Brunk will be doing the concussion baseline exams, and Myla Bandel will be doing the height, weight, vision, and vital signs testing.
