Physicals to Be Given on May 7
Cheyenne County Health Nurse Mila Bandel has asked that the following information regarding school sports physicals be distributed. The Cheyenne County Clinic and Health Department will be conducting sport physicals for the 2018-2019 school year at St. Francis Community High School on Monday, May 7. 2018. The physicals will be conducted by Dr. Dallas Walz and Tyler Raile, PA beginning at 7:30 a.m. and will be free of charge. If you child does not take part of the "during school" physicals and wants to make an appointment at the clinic, there will be a charge for a clinic visit.
A Pre-Participating Physical Evaluation and Concussion / Head Injury Release Form was sent home earlier with students. The form must be returned to the school completed and signed prior to their physical examination on May 7, 2018. Exams will NOT be given to students without the proper signed forms.
