Cheyenne County Clinic & Health Department Free Sports Physicals
On Tuesday, May 9, the Cheyenne County Clinic & Health Department will be giving free sports physicals to students in grades 5 - 11. They will start at 7:30 AM that morning. All students have been given a physical form and these must be completed and signed by a parent or guardian prior to receiving the physicals. If you forget to sign, your child will not be receiving a physical. It is important you sign on all designated lines (4 times). Feel free to send the copy back to school with your child anytime this week and give it to Mrs. Dinkel or leave it in the office. Attached are copies of the forms in case your child doesn't get you one.
